Name Correction Affidavit Submitted for Rectification of Official Records
Kamalanagar, June 9, 2026: In a move aimed at ensuring consistency in official identity records, a resident of Chawngte subdivision has formally submitted an affidavit seeking correction of a name discrepancy appearing in certain government-issued documents.
According to the affidavit, Smt. Padda Nondi Chakma,Daughter of Nilo Prasad Chakma and a resident of Ulsury village under Chawngte Police Station in Lawngtlai district, stated that her name had been inadvertently recorded incorrectly in her Aadhaar Card and Electoral Photo Identity Card (EPIC).
The deponent clarified that her correct name, as reflected in her Birth Certificate, is Padda Nondi Chakma, and that the variation found in the aforementioned documents resulted solely from a clerical or typographical error. She further affirmed that both versions of the name refer to the same individual and that there has been no change in her identity.
In the sworn declaration, Smt. Chakma stated that she is a bona fide citizen of India by birth and belongs to the Chakma Scheduled Tribe community. She also affirmed that all statements made in the affidavit are true and correct to the best of her knowledge and belief.
The affidavit was duly executed and affirmed before the Judicial Officer of the Chakma Autonomous District Council Court, Kamalanagar, on June 1, 2026. Such affidavits are generally submitted to facilitate the correction and uniformity of personal details across official records and government documents.
The document is expected to assist the concerned authorities in updating and harmonizing the applicant's identity records, thereby preventing future administrative complications arising from inconsistencies in personal information.
